About Us
CHC Medical is a busy, GP-led medical practice located opposite Coffs Harbour Hospital within the Specialist Medical Centre. We work closely with allied health professionals and pharmacists to provide coordinated, patient-centred care to our community.
We are seeking a HMR-accredited Pharmacist to join our collaborative healthcare team and deliver high-quality Home Medicines Reviews (HMRs) to our patients.
The Role
* Conduct Home Medicines Reviews in patients' homes or via telehealth
* Liaise with GPs, nurses, and allied health providers to optimise medication management
* Provide clear, actionable recommendations to improve safety and therapeutic outcomes
* Maintain accurate, timely documentation and comply with program requirements
* Manage your own schedule to ensure timely follow-up and reporting
* Provide your own transport to attend patient homes across the Coffs Harbour region (travel reimbursed as per agreement)

About You
* Current AHPRA registration as a pharmacist
* HMR accreditation (AACP or equivalent)
* Strong clinical knowledge and communication skills
* Ability to work independently and manage your own time effectively
* Commitment to patient-centred care
* Reliable vehicle and willingness to travel to patient homes
