Job Description:
We are seeking a seasoned leader to lead our Home Care Services team. As a Lead, you will be responsible for influencing strategy, growth and performance of case management services making a real difference in clients' lives while driving operational excellence.
Key Responsibilities:
* Lead mentor and develop a team of case managers to deliver exceptional client outcomes.
* Oversee the operational performance compliance and service quality across care plans.
* Partner with senior leaders to identify opportunities for innovation and improvement.
* Drive team capability through coaching professional development and recruitment strategies.
* Manage key performance metrics to ensure sustainable growth.
* Build strong relationships with clients families and sector stakeholders to promote high-quality responsive service models.
Requirements:
* 3+ years leadership experience in Home Care Services.
* Experience managing remote and multi-site teams and driving operational performance.
* Strong understanding of Support at Home program budgets utilisation compliance and legislation.
* Experience in change management organisational growth and service transformation.
* High-level communication leadership and problem-solving skills.
* National Police Check (mandatory)
* Valid Driver Licence
* Qualification in Health Community Services Business or Leadership.
* Ability to Travel Inter-State
* Experience in workforce planning coaching or organisational development.
