Job Description:
We are seeking a Registered Nurse with expertise in Chronic Disease Management to join our team. The successful candidate will collaborate with a multidisciplinary team to provide high-quality care to patients.
Key Responsibilities:
* Deliver patient-centered care and develop effective management plans for chronic diseases.
* Collaborate with healthcare professionals to ensure comprehensive care and optimal outcomes.
* Conduct thorough assessments, diagnose, and manage chronic conditions, including diabetes, cardiovascular disease, and respiratory illnesses.
* Maintain accurate records and communicate effectively with patients, families, and healthcare teams.
Requirements:
* Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
* Demonstrated experience in chronic disease management and specialist nursing practices.
* Excellent communication and interpersonal skills.
* Australian citizenship or permanent residency or legal eligibility to work in Australia.
