Clinical Care Manager Role
Delivering high-quality, resident-centric care in a timely and compassionate manner is essential for empowering residents to make informed choices and maintain control over their support needs.
You will be responsible for:
* Developing and implementing comprehensive care plans,
* Conducting thorough clinical assessments,
* Monitoring medication management and ensuring accurate recording of information in accordance with organisational and legislative requirements,
* Leading the development and submission of AN-AAC applications,
* Responsible for managing comments and complaints in collaboration with the Home Manager.
Key Responsibilities
* Demonstrated expertise in aged care legislation;
* Knowledge of the Australian National Aged Care Classification (ANACC) funding model;
About You
* Registered Nurse with current registration with the Australian Health Practitioner Regulation Agency (AHPRA);
* Aged care experience as a Registered Nurse;
* Experience in managing quality clinical care in a highly regulated environment;
* Commitment to contemporary aged care nursing practice;
* Ability to lead and support a multi-disciplinary team;
* Strong clinical skills and ability to coach and mentor others;
* Effective communication and time management skills;
* Alignment with the philosophy of resident-focused care which supports dignity, independence, and informed decision-making;
