We're looking for an experienced and motivated Social Worker to join our Cessnock Kurri Kurri Social Work Team, based at Kurri Kurri Hospital Multipurpose and Allied Health Centre.
In this role, you'll be a key member of our skilled multidisciplinary team, working closely with medical and allied health colleagues to support patients across the medical ward and rehabilitation unit.
What you'll do
* Deliver psychosocial interventions and supports to patients and their families
* Undertake psychosocial assessments and facilitate family meetings
* Complete Guardianship applications and support NDIS-related processes
* Receive and manage referrals through the Electronic Journey Board
* Contribute to patient-centred care in both inpatient and rehab settings
What we're looking for
- We're seeking a skilled, independent practitioner who can thrive with minimal supervision and bring strong psychosocial assessment skills. You'll also need:
A current driver's licence and willingness to travel across the region for meetings
- Flexibility to contribute to an after-hours social work roster when required
- A commitment to professional growth and collaboration

Requirements:
- Must hold as a minimum a bachelor degree in social work which provides eligibility for membership of the Australian Association of Social Workers, or other qualification deemed equivalent by the employer

* To be eligible for permanent employment in this position you must be an Australian Citizen, or a permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport.
