A qualified and experienced Clinical Case Manager is required for a North Queensland Community Provider.

Your new role

As the Clinical Case Manager, you will be responsible for coordinating and managing personalised care plans for participants who wish to remain living independently at home. This includes assessing needs, developing tailored care plans, managing budgets, and ensuring high-quality, client-focused services.

Key Responsibilities

* Client Assessment: Conduct comprehensive assessments and determine eligibility for services.
* Care Planning: Develop and review individualised care plans with clients, families, and health professionals.
* Budget Management: Manage budgets and provide clear guidance on fund allocation.
* Service Coordination: Organise and monitor delivery of care services, liaising with providers and stakeholders.
* Client Advocacy: Support clients' rights and preferences; assist in navigating aged care systems.
* Monitoring & Evaluation: Track progress, adjust care plans, and ensure ongoing relevance.
* Compliance & Reporting: Maintain accurate records and ensure adherence to Aged Care Quality Standards.
* Crisis Management: Respond to emergencies and document incidents per policy.
* Education & Support: Guide clients and families on care management, independence, and safety.

What you'll need to succeed

To succeed in this role, you will need:

* A minimum qualification in a Diploma of Nursing.
* Previous experience working within Community Aged Care and in writing care plans is desirable.
* The ability to work well within a team.
* A positive approach and communication style.
* Current valid Driving Licence with own vehicle.
* Current Working with Children's Check.
* National Police check or ability to obtain.
* First Aid & CPR
* NDIS Worker orientation module
* NDIS Worker screening check
