The Opportunity:
We are seeking an experienced person to work as the Chronic Conditions Aboriginal Health Practitioner/Nurse with our community members in the Western sandy desert.
This role is responsible for providing expert care and support to Aboriginal people with, or at risk of, chronic conditions. This role provides specialised chronic conditions support, coordination, and education to clinic nurses across the Ngaanyatjarra Lands and acts as a key liaison for visiting specialist to ensure integrated care. A key purpose is to develop and maintain robust clinical systems for recalls, and to work collaboratively within the multidisciplinary team to improve health outcomes and directly contribute to achieving national KPI targets.
About You:
To qualify for this role you need to possess the following:
* Be registered with AHPRA within relevant discipline
* Previous extensive experience in Aboriginal Health or in a remote Primary Health Care setting, with knowledge and experience in Preventable Chronic Disease frameworks and associated management systems and strategies
* Highly developed interpersonal and communication skills to work across a multidisciplinary workforce comprising people from language and culturally diverse backgrounds
* Demonstrated understanding of issues affecting Aboriginal health, the principles of
* Primary Health Care and legislation governing work practice
* Demonstrated ability to promote and manage change in a complex remote practice setting
* Demonstrated knowledge and commitment to Quality Improvement, evidence-based practice, research, education, and professional development
* Proficient in the application of a range of information technologies including patient information and other data management systems
* A current manual drivers' license
* A National Police clearance and WA Working with Children's check or willingness to obtain.
It would be advantageous if you possess the following in addition to the requirements listed above.
* Post graduate qualifications in chronic disease management, diabetes education, rural and remote health, or another relevant clinical discipline
* Experience in delivering chronic disease self-management training and information
* Previous experience in care coordination
* Experience in working in a primary health setting
