About the role:
This is a flexible hybrid position based in Canberra, ideal for a Social Worker who is passionate about helping older Australians age safely and confidently at home. You'll work directly with Home Care Package clients, delivering both in-home visits and remote support to assess emotional, social, and functional needs.
From conducting psychosocial assessments to providing short-term emotional support, guiding families through change, and connecting clients with local community resources, your role will be deeply personal and impactful. You'll collaborate closely with our allied health team and referring care managers to develop clear, tailored care plans that reflect each client's goals and values.
This is a truly holistic role where your insights and compassion will help clients feel seen, safe, and supported—exactly what aging in place should look like.
What you'll need:
* Experience in conducting assessments and creating personalized care plans
* Skills in coordinating with healthcare professionals and delivering therapeutic interventions
* Strong communication, empathy, and interpersonal skills
* Familiarity with aged care and understanding of the needs of elderly clients
* Ability to work independently and in a hybrid work environment
* Bachelor's degree in Social Work
* Registration with the Australian Association of Social Workers (AASW)
* Previous experience in a similar role is a plus
* Valid police check, WWWVP clearance, and driver's license
