Remuneration:
You will receive a competitive remuneration package value up to $132,567 p.a., comprising salary between $79,051 - $116,189 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP3) (Permanent Full Time, 76 hours p.f.) * The Transition Care Program (TCP) is a jointly funded initiative of the Commonwealth Department of Health and Ageing and Queensland Health, offering time‐limited and goal oriented flexible rehabilitation care packages to older persons who have had a recent acute or sub‐acute hospital admission and live within the Wide Bay Health Service region. It is aimed at older clients, but age alone is not an eligibility criteria.
* Deliver effective rehabilitation occupational therapy clinical services to Transition Care clients:
o maximise client function in activities of daily living
o maximise functional recovery
o supports timely transition to the Transition Care program
o supports clients' abilities to stay in their own home
