About the role
The Clinical Nurse Consultant is a senior Registered Nurse who will provide clinical nursing leadership within the Preventative Integrated Care Service(PICS). The position is accountable/responsible for the clinical triaging and referral co-ordination for the PICS model of care and will be required to work collaboratively in consultation with the Nurse Practitioners and Senior Medical Officer's to ensure safe delivery of nursing care to chronic condition patients who are accepted to the PIC service.
About you
- Contemporary clinical knowledge in the areas of specialised sub-acute and chronic disease management, in a community setting in collaboration with individuals, peers and the healthcare team to achieve improved health outcomes across Preventative Health Services.
- Leadership in the coordination of nursing and provision of health services across the continuum of care, delivering competent nursing care in complex situations and ability to coach patients to adopt self-management strategies.
- Ability to adopt digital technology and utilise digital care solutions and telehealth as part of delivering and co-ordinating care closer to home.
- Advanced level delivery of interpersonal (problem solving/conflict resolution), communication (verbal/written) and time management skills whilst demonstrating autonomous, sound, high level conceptual and critical thinking capabilities.
**Mandatory Requirements**:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
