We are seeking a highly skilled and experienced Clinical Nurse to join our dynamic team in Mount Isa, North West Queensland. As a vital member of the North West Hospital and Health Service (NWHHS) Diabetes Service team, you will play a key role in supporting the delivery of high-quality diabetes care and education across the NWHHS catchment.
About the Role
This is an exciting opportunity for a motivated and enthusiastic nurse to work in a challenging and rewarding environment. You will be part of a dynamic, interdisciplinary group based in Mount Isa, operating from the Community and Primary Health Care (CPHC) building and maintaining strong links with various CPHC services.
The Clinical Nurse - Diabetes Educator plays a vital role in supporting the delivery of high-quality diabetes care and education across the NWHHS catchment, including First Nations communities.
Key Responsibilities
* Provide high-quality diabetes care and education to patients across the NWHHS catchment.
* Work collaboratively with the interdisciplinary team to deliver integrated models of care designed to create a seamless healthcare continuum.
* Develop and implement effective strategies to improve patient outcomes and engagement in their care.
Requirements
To be successful in this role, you will require:
* A relevant tertiary degree qualification in nursing or a related field.
* Appropriate registration/licences to practice as a nurse in Australia.
* A C Class Motor Vehicle Licence.
* Pre-employment screening, including Criminal History checks.
* Evidence of vaccination or immunity to specified diseases.
