About The Role

Are you passionate about improving health outcomes for Aboriginal and Torres Strait Islander communities? The Diabetes Alliance Program Plus (DAP+) team is seeking a dedicated Aboriginal Health Worker/Practitioner to deliver culturally appropriate diabetes care across diverse communities in the Hunter New England region.

The role offers opportunities to connect with urban, rural and remote communities through the DAP+ Medibus and the integrated care model between diabetes specialists and the primary care sector. You'll play a vital part in supporting patients within Aboriginal Medical Services and GP practices, while travelling throughout the Hunter New England region.

As an Aboriginal Health Worker/Practitioner you'll build strong, meaningful relationships with Aboriginal Medical Services, Local Aboriginal Land Councils, GP practices, healthcare providers, community Elders, and members. Every day offers variety—combining clinical and non-clinical tasks, overseeing and supporting the DAP+ Aboriginal Reference Group, and yarning for cultural connection to improve health outcomes for Aboriginal people.


In This Rewarding Role, You Will

* Provide care within your scope of practice, guided by the HNE Health Aboriginal Health Workers and Practitioners Framework.
* Engage with local Aboriginal communities to design and implement culturally safe health strategies.
* Build strong partnerships with Aboriginal Medical Services (AMS), ACCHOs, LALCs, and primary care providers.
* Support individuals and families to understand clinical interventions and make informed health decisions.
* Collaborate within a multidisciplinary team and lead the Aboriginal Reference Group to ensure cultural safety in program priorities.
* Manage essential administrative tasks, including data collection and documentation.


Where You'll Be Working

You will be based at Wisteria House, James Fletcher Campus in Newcastle, working within a multidisciplinary team that includes endocrinologists, diabetes educators, managers, administration, and the DAP+ Aboriginal Reference Group.


Requirements

* Aboriginal Health Worker: Holds a minimum Certificate III qualification in Aboriginal Primary Health Care or a minimum Certificate III health qualification in the area of care in which the Aboriginal Health Worker works.
* Aboriginal Health Practitioner: Holds a Certificate IV in Aboriginal Primary Health Care (Practice) and a current authority to practice as an Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA).
* Eligible to drive in NSW and the ability to travel as required.

Additional Information

* To be eligible for employment you must be an Australian Citizen, permanent resident of Australia, a New Zealand citizen with a current New Zealand Passport, or hold an appropriate visa that allows you to live and work in Australia. Employment of a temporary visa holder will only be offered in line with the conditions and expiry date of your visa.
* An eligibility list will be created for future temporary full or part time vacancies.

* Determination 1 of 23 Aboriginal Health Worker classification – recognition of prior service ensures provisions of Recognition of Prior Years of Service that will influence the commencement of rate of pay for the successful candidate.
