Assessment Specialist - Aged Care

An exciting opportunity exists for a skilled and compassionate Assessment Specialist to join our team in the aged care sector.

This role will see you conducting thorough assessments to identify and meet the complex needs of clients, as well as referring them to appropriate services under the Aged Care Act.

Key responsibilities will include:

* Collaborating with clients, families, and multidisciplinary teams to deliver high-quality outcomes;
* Working collaboratively with colleagues to promote a supportive and positive team culture;

Additional key duties include:

* Conducting accurate and comprehensive assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services;
* Reviewing support plans to ensure they are meeting the evolving needs of clients;
* Maintaining currency of knowledge related to aged care services, both funded and non-funded;
* Contributing to continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes.

To succeed in this role, you will need:

* A tertiary qualification in a health-related discipline directly related to health, aged care, or a related specialist area;
* Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA);
* An Australian Driver's License and access to a registered, insured vehicle; and
* Minimum one year of demonstrated experience in Australia or overseas delivering services in aged care settings and/or to aged persons, including those living with dementia.
