About the Role
The primary function of this position is to deliver high-quality nursing care in a community-based setting.
This role involves coordinating and providing outreach nursing services to residents of residential aged care facilities and those living in the community.
* Nursing care will be delivered in accordance with relevant standards and policies, ensuring that clients receive person-centered care.
* Develop and maintain strong relationships with facility staff and other healthcare professionals to ensure seamless coordination of care.
Required Skills and Qualifications
To be successful in this role, you will require:
* A current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia.
* Strong communication and interpersonal skills, with the ability to build rapport with clients and their families.
* Ability to work independently and as part of a team, with a strong commitment to delivering high-quality care.
