Carer / Mentor

About the Role
We are seeking a compassionate, reliable, and motivated Carer / Mentor to support individuals living with disability to live more independent, confident, and fulfilling lives.
We are currently looking for someone with experience supporting clients in Supported Independent Living (SIL). The role also requires the capacity to work as a mentor, supporting individuals to challenge themselves and work towards their personal goals in line with NDIS or Department for Child Protection requirements.
This role goes beyond basic care. We are looking for someone who can build genuine connections, provide positive guidance, and act as a mentor while supporting day-to-day needs.
Key Responsibilities
* Providing person-centered disability support in the home and community
* Assisting with daily living tasks, including personal care, meal preparation, medication support, and domestic tasks as required
* Supporting community access and social participation
* Encouraging independence, confidence, and the development of life skills
* Providing mentoring, emotional support, and positive role modelling
* Implementing support plans and following care routines
* Maintaining clear communication with family members and / or support coordinators
* Completing basic documentation and progress notes
About You
You are patient, empathetic, and genuinely passionate about supporting others. You understand the importance of dignity, choice, and empowerment, and you consistently work from a relational-based model of support and care, always putting people first.
Essential Requirements
* Experience as a Disability Support Worker, Carer, or similar
* Strong communication and interpersonal skills
* Ability to build trust and rapport
* Current NDIS Worker Screening Check
* Working With Children Check (if applicable)
* Current First Aid & CPR
* Valid driver's licence and reliable vehicle
Desirable
* Certificate III or IV in Disability or Individual Support
* Relevant certifications such as medication training and infection control
* Experience with caring, mentoring, coaching, and behavioural support
* Understanding of the NDIS and person-centered practice
