Community Health Care Professional
We are seeking a skilled and compassionate Community Registered Nurse to join our team. As a key member of our multidisciplinary team, you will provide holistic and flexible nursing services to individuals living in the community.
Your primary focus will be on assessing customers' needs, implementing preventative-based approaches to healthcare, and providing expert advice and services within your scope of practice. You will maintain accurate and up-to-date customer records, including care plans, progress notes, customer reviews, outcome measures, incidents, and consent.
This is a mobile role, allowing you to work flexibly and autonomously. You will spend most of your time meeting with customers in their homes, assessing their needs, and delivering high-quality care. Administrative tasks can be completed at our office or from home, giving you the flexibility to plan your schedule according to your needs.
This role supports our Rockhampton region, and we welcome applications from both experienced nurses and new graduates. About You:
* You must have a tertiary qualification in Nursing and be currently registered with AHPRA - Australian Health Practitioner Regulation Agency.
* Experience working within the health, community, or social service sector, particularly with ageing and/or disability, is highly desirable.
* You will possess exceptional clinical skills, including assessment, care plan implementation, wound care and management, medication support, continence assessments and management, clinical monitoring, nursing advice and recommendations, and general nursing duties.
* Effective communication skills, ability to work collaboratively with the multi-disciplinary team, Suncare, and other organizations as necessary for provision of optimum customer care.
* Excellent time management and organization skills.
* A valid Driver's License and willingness to use your own vehicle for work.
* A current First Aid and CPR Certificate and National Police Check.
