The rehabilitation program is dynamic and multi-disciplinary and offers a bridge between a patient’s hospital stay and returning safely to the community. About the role: Reporting to the Nurse Unit Manager, your role as an Enrolled Nurse/Registered Nurse, some of your duties may involve: Assess individual patient/client needs, plan, implement and/or coordinate appropriate service delivery from a range of accepted options including other disciplines or agencies, Demonstrates knowledge and skill in rehabilitation assessment, care and routine procedures for rehabilitation nursing practices Recognises the importance of evidence based practice, Provide health promotion and education, to patients/clients or groups and carers to improve the health outcomes of individual. Please note: This position is multi-classed as either Enrolled Nurse or Registered Nurse and classification will be determined by qualifications and/or experience.
