Clinical Care Partner Responsibilities:
* Developing comprehensive care plans
* Collaborating effectively with multidisciplinary teams to achieve optimal client outcomes
* Monitoring and addressing client needs in a timely manner
Requirements and Qualifications:
* AHPRA registration as a Registered Nurse or Allied Health professional
* Demonstrated strong communication and interpersonal skills
* Possession of an Australian Driver's Licence
