We are seeking a passionate and skilled Care Navigator to join our Child and Adolescent Mental Health Service (CAMHS) and support young people with complex neurodevelopmental and mental health needs.

About the Opportunity

As the Care Navigator within CAMHS, you will play a pivotal role in improving access, coordination, and continuity of care for children and young people with co-morbid neurodevelopmental, behavioural, and mental health presentations.

Working closely with families, multidisciplinary teams, community partners, NGOs, and the disability sector, you will help bridge service gaps and create clear, achievable pathways of care. This role is central to strengthening collaboration across health, education, and community systems and ensuring families feel supported, informed, and empowered.

In this role, you will:

* Act as a key point of contact for families attending CAMHS, supporting them to navigate complex service systems.
* Collaborate with families and multidisciplinary teams to develop and implement timely, achievable, person-centred care plans.
* Coordinate and facilitate multidisciplinary team (MDT) processes, including collating information, organising meetings, and tracking agreed goals.
* Develop and maintain effective service pathways between CCLHD, NDIS, Early Childhood Early Intervention (ECEI), CMOs, NGOs, schools, and private providers.
* Provide consultation and liaison support to CCLHD staff regarding best practice, referral options, and available supports for neurodevelopmental and mental health needs.
* Contribute to service improvement projects, including the development of referral tools, service maps, and consumer-focused information resources.
* Support system-level capability building by improving knowledge and confidence in working with neurodiversity, recovery-oriented practice, and NDIS processes.

* Occupational Therapist Level 3
* Physiotherapist Level 3
* Social Worker Level 3
* Speech Pathologist Level 3
* Clinical Psychologist
* Senior Psychologist

About You

We are looking for a motivated and compassionate clinician who is confident working across systems and disciplines. You will bring strong communication skills, sound clinical judgement, and a genuine commitment to recovery-oriented, family-centred care.

You will have:

* Degree qualifications in your relevant clinical discipline and current professional registration (e.g. AHPRA or relevant professional body), with a minimum of 3 years post-qualification clinical experience.
* Demonstrated experience working within Child and Adolescent Mental Health Services or a mental health setting.
* Strong understanding of Recovery Oriented Mental Health Practice.
* Experience in case management and developing, implementing, and reviewing client-centred support plans.
* Proven ability to work independently and collaboratively within a multidisciplinary team.
* Experience working with community partners, including GPs, schools, NGOs, CMOs, and disability services.
* Current Working With Children Check, National Police Check, Category A immunisation compliance, and a current Australian Driver's Licence.
