About The Role
This position is responsible for leading case management and the delivery of person-centred home and community care services. You will facilitate the delivery of quality services to support clients to live in their homes longer and stay connected to their community. The role acts as the central contact for clients and manages care planning based on individual needs and available funding.
Your Key Accountabilities
* Advocate for client choice, informed decision-making, and flexibility, ensuring the client's voice is central to service provision.
* Plan services tailored to meet individual needs within funding constraints.
* Identify and escalate vulnerable, at-risk, or complex care clients.
* Participate in governance, continuous improvement, and risk management activities to ensure quality and safety.
* Adhere to Uniting's values, policies, procedures, and legislative requirements.
* Maintain high standards of conduct and work performance aligned with Uniting's values.
* Promote collaboration across departments to deliver seamless client services.
* Engage in performance management and review processes.
* Utilise Uniting's systems and practice tools effectively.
* Ensure safety and wellbeing of yourself, clients, and colleagues, working in accordance with policies.
* Contribute to an inclusive, safe, and supportive work environment that celebrates diversity.
* Seek personal development opportunities to stay current with industry changes.
* Coordinate service delivery activities with team members.
* Willingness to travel between community sites and client locations.
Your Qualifications And Experience
* Certificate IV in Aged Care or Diploma of Community Service; higher qualifications in relevant fields are highly regarded.
* Experience with client management systems.
* Current Australian driver's license, own vehicle, and comprehensive insurance.
* Current First Aid Certificate.
