HARP Case Manager Role
The HARP Case Manager is a key position within the Hospital Admissions Risk Program Complex Care (HARP CC) service. This service operates under the Department of Health and Human Services guidelines, focusing on providing hospital substitution and diversion by supporting individuals in their communities, ambulatory settings, and homes.
Responsibilities
* Delivering short-term case management and care coordination to clients with complex psychosocial issues and chronic health conditions that pose a risk of potentially avoidable hospital presentations.
Care Delivery Model
You will work within a multidisciplinary team, utilizing an interdisciplinary approach to care. Your daily activities will be overseen by site-based HIP Complex Care Clinicians (HIP CCC). You will develop client-centered SMART goals in collaboration with the multidisciplinary team, ensuring a holistic approach to achieving these goals outlined in the client's care plan.
Requirements
To be successful in this role, you must hold an approved degree in Social Work or another Allied Health discipline. You should have four years of experience post-qualification, a Victorian Driver's Licence, and a Working with Children's Check. Advanced skills and experience are highly regarded, as evidenced by a demonstrated commitment to professional development.
