Role

South Western Sydney Local Health District's Transitional Aged Care Program (TACP) is seeking a Level 1/2 Occupational Therapist to join its community‐based multidisciplinary team. This is an excellent opportunity to be part of a unique program focused on the reablement of older adults living in the community. TACP supports individuals transitioning from hospital back into the community, with a strong emphasis on improving health outcomes, maximising independence and enhancing quality of life. In this role, you will work closely with the multidisciplinary team and relevant stakeholders to deliver holistic, coordinated care for TACP clients and their families within a community setting.

Are You the Right Fit?

You are an Occupational Therapist at Level 1 or 2 who understands the importance of timely, coordinated interventions in community aged care settings. You bring strong clinical reasoning skills and the ability to manage a diverse caseload while contributing positively to a multidisciplinary team environment. You recognise the essential role occupational therapy plays in discharge planning and patient safety and are committed to delivering high‐quality, evidence‐based care in clients' homes. Your contribution will directly support team efficiency and effectiveness, helping to prevent service gaps, reduce risk to patient outcomes, and support sustainable workloads for the broader team.

Selection Criteria

To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.

* Recognised Degree in Occupational Therapy or other qualification deemed appropriate by the employer, with registration with the Australian Health Practitioner Regulation Agency (AHPRA)
* Demonstrated clinical experience in aged care and or rehabilitation
* Ability to work independently and as part of a multidisciplinary team
* Evidence of knowledge and commitment to the application of quality improvement and evidence based practice
* Excellent written and verbal communication skills and computer literacy in a range of programs
* Applicants must possess an Australian C-Class driver\'s license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner\'s licenses are not permitted.
