Job Summary
The Care Partner will work collaboratively with clients, families, and internal teams to design and implement personalized care plans that cater to the unique needs and goals of each individual. Key responsibilities include providing daily living support, developing and implementing care plans, and promoting wellness and reablement initiatives.
Key Responsibilities:
* Deliver exceptional care and support to clients in their homes, ensuring their physical, emotional, and social needs are met.
* Conduct thorough assessments to identify client strengths, weaknesses, and goals, informing the development of tailored care plans.
* Collaborate with multidisciplinary teams to ensure seamless care coordination and outcomes.

Required Skills and Qualifications:
* Diploma of Nursing or higher qualification.
* Certificate III in Individual Support or equivalent (if applicable for direct care duties).
* Minimum 1 year of experience in aged care, home care, or community services.
