About The Role
In this exciting growing role, you will report to our Home Care Manager, managing a case load of Home Care Package Residents in our villages and ensuring the smooth running of end-to-end customer experience. You will focus on the wellbeing of older people receiving our Home Care Packages, working across two of our villages: Nellie Melba Retirement Village and Bert Newton Retirement Village.
You will act on new leads and enquiries, assess resident care needs, develop care plans, monitor home care budgets, and remain within allocated funding. You will collaborate with care staff, monitor care provided, and focus on customer satisfaction and maximizing outcomes for residents with a Home Care package.
This position involves a hybrid work setup, including office, home, and village-based responsibilities, predominantly focused on supporting residents across various villages in the southeastern suburbs of Melbourne.
About You
* Case Management experience within aged care, or an understanding of Consumer Directed Care within Home Care Packages, as well as an understanding of HCP guidelines and standards
* Tertiary qualification in a health‐related field
* Nursing degree is highly desirable
* Have a warm and friendly approach to build lasting relationships with our residents and their families
* Exceptional organisation, problem solving, computing, client database management and time management skills
* Excellent written, verbal, and numerical communication skills
* Current driver's licence
