We're seeking a Comprehensive Support Associate to deliver person-centred care, working with clients, carers, and teams to support independence, health, and quality of life.
The Opportunity
* Complete holistic assessments and co-design care plans aligned to client goals and clinical recommendations.
* Coordinate and monitor services under CHSP, Support at Home, QCSS, and NDIS.
* Help clients and carers navigate systems, provide informed consent, and share feedback.
* Collaborate with internal/external clinicians for complex needs and referrals.
* Keep accurate records and report outcomes to funding/quality frameworks.
* Build strong, respectful relationships with clients, families, and community stakeholders.
* Travel to outreach locations as required.
Nice to Have:
* Familiarity with Support at Home, Home Care Packages, CHSP, QCSS, and NDIS.
* Clinical background (e.g., RN or Allied Health) for the Clinical Care Partner stream.
Mandatory Requirements:
* Current First Aid & CPR.
* Current Australian driver's license.
* National Police Check and NDIS Worker Screening (or ability to obtain).
* Unrestricted Australian working rights.
* Clinical Care Partner only: Current AHPRA registration (discipline-relevant).
