About Our Senior Mental Health Clinician Role
Job Description
This is a part-time position located in Northam that focuses on providing care and support to individuals living with severe mental health conditions. The ideal candidate will work collaboratively within our Clinical Care Coordination stream.
Main Responsibilities
* Deliver high-quality, client-centered care coordination
* Conduct comprehensive assessments, triage, and risk assessments
* Develop and implement recovery-oriented care plans with clients and their carers
* Provide clinical case management, crisis intervention, and therapeutic support as needed
* Offer guidance and supervision to team members delivering coordinated care
* Contribute to service development and continuous quality improvement initiatives
The successful candidate will be an experienced Registered Nurse with a strong background in mental health and a passion for delivering quality healthcare services.
