About the Role:

As a Capecare Clinical Care Coordinator, you will lead the delivery of safe, high-quality, person‐centred care, ensuring services are delivered in partnership with consumers and aligned with best practice. You will establish and maintain strong systems and support staff to deliver care that is compliant with the ACQSC Quality Standards, reflects Capecare's Clinical Governance Framework, and brings Capecare's Purpose, Vision and Values to life.
This role works alongside a team of Clinical Care Coordinators and reports to the Clinical Manager.
Your key responsibilities will include:
* Attend 'huddle' and handover to ensure accurate and timely communication between shifts and any follow‐up actions required are identified and completed.
* Support and facilitate the Registered and Enrolled Nurses to ensure complete holistic, individual assessment of residents.
* Ensure that consumers and nominated representatives are involved in planning care and agreed services and care plan consultations/reviews are undertaken at the agreed schedule.
* Coordinate Resident of the Day Assessment schedule.
* Communicate relevant information to nominated representatives as escalated by Registered Nurses, for example open disclosure discussions.
* Monitor medication management system and support staff in its use.
* Escalate to the Clinical Manager any incident or change in a consumer's health status that may indicate a need for increased risk management strategies, e.g. reportable infectious diseases, critical incidents, compulsory reporting events.
* Ensure all AN-ACC reviews are undertaken on the required schedule.
* First point of contact for resident family and representatives where an issue/concern cannot be resolved by the RN.
* Support staff in the required use of the Care Management System and Medication Management System.
* Ensure all clinical documentation is completed accurately and on time.
* Ensure accurate and comprehensive investigations of consumer incidents are completed.
* Understand the reporting requirements of SIRS and ensure the timely reporting of incidents as required.
Education and Training:
* Take personal responsibility for updating knowledge base of contemporary nursing issues.
* Attend mandatory training and proactively pursue own professional development.
* Maintain and update knowledge of aged care quality standards, the Capecare Clinical Governance Framework and all associated practices.
* Develop and deliver both formal and informal education to nurses, carers and other staff as required including On‐boarding and Toolbox sessions.
* Mentor nurses to assist the acquisition of new skills and consolidate their clinical knowledge and practice.
Leadership and Staff Management:
* Provide clinical leadership to the Registered Nurses and Care team.
* As directed participate in the recruitment and probation reviews of Registered and Enrolled Nurses, Team Leaders and Care staff.
* Actively participate in the onboarding of Registered Nurses via the Buddy program.
* Oversee the daily allocation of staff to ensure an appropriate skill mix.
* Provide constructive guidance and feedback to Registered Nurses, Enrolled Nurses and Carers and ensure immediate action is taken to rectify poor and incorrect care practices by staff under their supervision.
* Ensure appropriate allocation and supervision of work‐experience/ student and Capecare Trainee placements.
* Assist with the completion and investigation of staff incident forms.
Quality:
* Actively contribute to Capecare's Plan for Continuous Improvement.
* Undertake data collection for monthly audits (internal and external) and clinical indicators, including the National Quality Indicator Program.
* As directed participate in the development and delivery of action plans for audits, risk reports, risk registers.
* Identify potential breaches of regulatory compliance and elevate to the Residential Manager.
To be successful in this role, you'll need:
* Minimum of 2 years of clinical leadership experience.
* AHPRA Registration.
* Demonstrated senior leadership with experience in aged care or a related field.
* Demonstrated knowledge of the Aged Care Act and Standards.
