About The Role
* Provide advanced nursing assessment and acute clinical care directly to residents in Residential Aged Care Facilities, reducing avoidable hospital presentations.
* Work autonomously and collaboratively with GPs, Geriatricians, and community services to deliver timely, person-centred care.
* Triage referrals, conduct face-to-face assessments, and support early intervention strategies within a dynamic, consultative nursing role.
* Maintain clear and timely communication with key stakeholders through telehealth, EMR, and written documentation to support integrated care planning.
Essential
* Post graduate qualifications in critical care
* Minimum 5 years' experience post registration
* Highly developed assessment, clinical reasoning, problem solving and prioritisation skills
* Strong interpersonal and communication skills both written and verbal
* High level of reliability and professional conduct
