The role of Mental Health Nurse is an integral part of our Continuing Care Team.
We are seeking a skilled professional to undertake comprehensive mental status assessments and reviews of clients in our care, establishing recovery goals in consultation with them, and supporting progress towards these goals within a case management framework.
Responsibilities
* Conduct thorough mental status assessments and client reviews, identifying key areas for improvement and development.
* Collaborate with clients and the multidisciplinary team to establish and pursue recovery goals, ensuring a tailored approach to each individual's needs.
* Participate in case management activities, contributing to collaborative care planning and facilitating effective communication among team members.
Requirements
* Substantial experience working within the Mental Health field, specifically in a Community Mental Health Setting.
* Proficiency in the Victorian Mental Health Act 2022, as a requirement for professional registration with AHPRA.
* Effective communication and interpersonal skills, enabling respectful and collaborative interactions with consumers and the multidisciplinary team.
* Strong documentation and record-keeping skills, ensuring accurate and timely information for case management purposes.
