Join Our Team: Youth Mentor- Staying On Track
Staying On Track (SoT) will improve community safety and reduce reoffending by bridging the transition to the community when young people are released from detention, including 12-months of support with at least 6-months of intensive supervision to help youth offenders turn their lives away from crime. A Youth Justice case plan for a young person, known as a Service Response Plan (SRP), identifies individual risk factors across key evidence-based domains, the young person's areas of need and required intensity of response.
The Staying On Track program will work collaboratively with Youth Justice to develop, deliver and support departmental service provision by delivering the practical, cultural, social support and mentoring components of a young person's SRP.
The Youth Mentor plays a key frontline role in supporting young people (YP) exiting youth detention to successfully reintegrate into their families, communities, and positive life pathways. Using a trauma-informed, culturally responsive, and youth-led approach, Youth Mentors build consistent, positive relationships with YP before and after release. Utilise program model and goal action process to implement, monitor and evaluate strategies to achieve positive change in young people. They provide practical and emotional support, coordinate service responses, strengthen cultural and community connections, and assist young people to achieve the goals set out in their Service Response Plan (SRP).

Qualifications:
Essential
* Relevant qualification in youth work, community services, social work, or related field, or equivalent experience.
* Demonstrated experience working with young people in the youth justice, child protection, or community services sector.
