Our client is seeking an experienced and clinically trained Complex Case Manager to support clients and their families through high-quality, trauma-informed case management. In this role, you will manage a caseload of clients with complex psychosocial and health needs, providing clinical expertise, coordinated support, and guidance in navigating systems and external services.
The successful applicant will play a key role in enhancing client wellbeing, by applying the clients Wellbeing Model, and ensuring consistent, high-standard case management practices across the Client Support Program.
The Duties:
* Provide specialised, trauma-informed clinical case management to clients with complex needs.
* Conduct intake assessments, risk assessments, and case reviews.
* Act as a primary point of contact for allocated clients, coordinating access to mental health, medical and external support services.
* Escalate high-risk client issues and work closely with Senior Clinicians and Assistant Directors.
* Build strong relationships across all business areas, including ex-service organisations, and community providers and Federal Government Agencies.
* Prepare high-level documentation, briefs and client summaries.
* Support colleagues with clinical advice, coaching and guidance for managing challenging presentations.
* Represent the program through group presentations and stakeholder engagement as required.
Essential Requirements:
* Tertiary qualifications (Bachelor or above) in Nursing, Psychology, Occupational Therapy or Social Work.
* Demonstrated experience supporting people with complex and/or co-morbid needs.
* Strong understanding of disability impacts and capacity-building support models.
* Excellent communication and stakeholder engagement skills.
* Experience in assessment, triage and working within structured case management frameworks (including telephone-based work).
