Job Title
As a Clinical Care Coordinator Flexible, you will be part of our team dedicated to delivering high-quality, evidence-based clinical care services for children and youth. Our focus is on early intervention and support for young people up to the age of 25.
The role involves addressing service gaps by enhancing accessibility and integrating primary mental health care, psychosocial support, and early intervention. You will promote holistic wellbeing by considering broader determinants of health, fostering collaboration among providers, and ensuring a sustainable, adaptable service responsive to community needs.
Key Responsibilities
* Contribute to the multidisciplinary consortia team in delivering care coordination services for children and youth.
* Address service gaps by enhancing accessibility and integrating primary mental health care, psychosocial support, and early intervention.
* Promote holistic wellbeing by considering broader determinants of health, fostering collaboration among providers, and ensuring a sustainable, adaptable service responsive to community needs.
Requirements
* AHPRA registration as an Occupational Therapist or Mental Health Nurse, or General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist, or AASW registration/eligibility as a Social Worker, or Level 2 ACA registration/eligibility.
* Knowledge and understanding of the theory underpinning evidence-based interventions and clinical practice in the areas of mental health and alcohol and drug use.
* Highly developed communication (including written) and interpersonal skills.
* Ability to engage with people, advocate on their behalf, and establish and maintain effective relationships with staff, clients, families & friends and stakeholders.
* Experience in the use of electronic client/customer management systems, ability to use a desktop computer, and experience with Microsoft Office applications.
