Overview
The Community Transition Care Program offers short‐term, goal‐oriented support for older people transitioning from acute or sub‐acute hospital settings to the home environment. The team is dynamic and multidisciplinary, supporting clients to optimise functioning and independence.
Role
The TCP Clinical Nurse works closely with the TCP Manager to triage referrals and assess clients' current care needs. The role attends initial home visits to identify needs of older persons and their carers, and provides clinical expertise, support, and advice to the multidisciplinary team in managing complex nursing care needs including falls, pressure injury, wound management, cognitive impairment, medication, continence and pain. The Clinical Nurse may deliver short‐term direct nursing interventions or facilitate onward referrals, and liaises with external healthcare services to ensure coordinated care.
Responsibilities
The TCP Clinical Nurse ensures that risks and barriers to safe discharge are addressed and appropriate supports are arranged. The role assists in planning and coordination of staff and resources, works autonomously within operational frameworks, and utilizes information technologies for effective client care and discharge planning.
Qualifications and Experience
* Highly developed clinical nursing knowledge, skills, and judgement with experience providing evidence‐based primary health care to older people with age‐related needs or cognitive impairment.
* Ability to work in a leadership partnership with the Nurse Manager to maintain a strong professional environment.
* Demonstrated ability to work autonomously and as part of a multidisciplinary team to provide effective and efficient client care and discharge planning.
Salary and Benefits
* Salary: $109,767 to $115,813 per annum (pro rata). The employer contributes 12% superannuation on top of this amount.
* Post Graduate Allowance of up to 10% based on qualification type.

Essential Criteria
* Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse.
* Current Tasmanian Working with Vulnerable People Registration – NDIS endorsed.
* Current driver's licence.
Desirable Criteria
* Relevant post‐registration nursing experience in sub‐acute/medical/community/aged care.
