Our team is seeking a dedicated and skilled Clinical Team Leader to join our senior nursing staff. This role is an excellent opportunity for a motivated individual with strong core nursing skills, an interest in psychosis and physical health, and a passion for service improvement.
Key Responsibilities:
* Coordinate patient-centered care by working collaboratively with the multi-disciplinary team (MDT) to deliver high-quality care plans.
* Liaise effectively with members of the MDT and partner organizations to ensure seamless care delivery.
* Administer medication safely, monitor side effects, and maintain safe custody of medicines, sharps, and clinical equipment.
* Lead and support junior staff in risk assessment and management, ensuring they have the necessary skills to provide safe care.
* Engage in one-to-one sessions with service users in a non-judgmental manner, promoting open and honest communication.
* Develop care plans that accurately reflect service user needs and relevant patient documentation.
This role requires strong leadership and communication skills, as well as the ability to work collaboratively with a diverse team.
