**The Role - Permanent, Full Time, SCHADS Level 4**

We currently have an opportunity to work with a complex client in the suburb of Broadview in a 2:1 male only placement.

The Individual Placement Coordinator (IPC) provides oversight of and works alongside a dedicated service delivery team to ensure quality and consistent support services are provided to clients of Baptist Care SA. The IPC supports one individual placement with complex needs client/s. The position will be based at a supervised home and at local Baptist Care SA offices. The IPC is required to build relationships with placement staff, clients, guardians, families and relevant external stakeholders, including Support Coordinators, NDIA Planners, OPA and Public Trustee (where relevant) to ensure a client centred and effective service is provided.

**Your responsibilities will include**:

- Provide direct support to clients (minimum 3 shifts per week) ensuring quality care is provided and to demonstrate best practice client support to support workers.
- Maintain consistent evidence of client requirements for support through appropriate use of ABC charts, sleep charts, detailed incident reports and action notes. Use evidence documents to support renewal of funding requests to the NDIA.
- Coordinate with internal and external stakeholders to ensure that client care is provided to a high standard, including support by allied health and community service providers.
- Ensure rosters are completed accurately and in accordance with the approved client Roster of Care / support schedule for allocated clients and that staff are rostered effectively in line with SCHADS award and their contract requirements.
- Complete all daily, weekly and monthly tasks outlined within the Coordinator Responsibilities template to a high standard and escalate any concerns with standard of care or placement issues immediately to Team Leaders and Managers as required.
- Ensure efficient service delivery which follows the Baptist Care SA Service Delivery Model (SDM) and meets client, NDIS and organisational requirements.

**What you'll bring**:
You will be a proactive, resilient, and passionate individual who thrives in challenging settings. A natural leader and communicator, you'll build strong and effective relationships with your client, Support Workers, Disability Pathways Team and stakeholders.

**You'll also have the following skills and experience**:

- Relevant tertiary qualification in the area of human services, for example disability, community services, psychology, nursing or related field.
- Significant previous experience in working with vulnerable and at-risk clients with disability and complex needs.
- Experience developing and implementing staff rosters and knowledge of the SCHADS award and how it applies to rostering of staff members effectively within award requirements and contractual obligations

- Evidence of current vaccinations including any booster requirements for COVID-19
- Full unrestricted South Australia Driver's Licence and access to own fully insured and roadworthy vehicle
- A valid NDIS Worker Screening Check
- A valid Department of Human Services Working With Children Check
- National Police Check
- First Aid / CPR
- Ability to work outside of normal office hours will be required
