We are seeking an experienced Clinical Lead to join our multidisciplinary team and deliver high-quality, culturally safe, and holistic care to our community. This role involves leading and supporting the delivery of community health programs, working collaboratively with GPs and allied health professionals, and providing clinical leadership to nursing staff.
Key Responsibilities:
* Lead and support delivery of community health programs such as smoking cessation, diabetes support, nutrition, and early intervention.
* Collaborate with GPs, Aboriginal Health Practitioners, Allied Health Professionals, and external partners to provide comprehensive care.
* Provide clinical leadership, mentoring, and support to nursing staff and Aboriginal Health Practitioners.
* Participate in case conferencing, quality improvement activities, and care planning meetings to ensure optimal patient outcomes.
* Empower patients through education and self-management guidance to promote healthy behaviors.
* Maintain accurate clinical documentation using Medical Director to ensure compliance with AHPRA, clinical guidelines, and organisational policies.
* Engage in ongoing professional development and clinical supervision to stay up-to-date with best practices.
