We are looking for a Link Worker (Social Worker or Allied Health professional) who is passionate about helping people reconnect with what matters most in their lives. This role focuses on social prescribing, supporting individuals to discover meaningful activities, build relationships, and connect with community opportunities that improve their wellbeing and quality of life.

This is not traditional case management. It's about listening deeply, understanding what is important to someone, and helping open doors to the possibilities that already exist in the community.

Your Role: Creating Meaningful Connections

As a Link Worker, you will work alongside people to explore their interests, goals and priorities. Through open conversations, you'll help individuals identify what gives their life meaning and connect them with opportunities that support their wellbeing.

Sometimes that might mean introducing someone to a local walking group, knitting circle, cooking class, nature-based activity, volunteering opportunity, or cultural group. Other times it may involve linking people to services, resources, or networks that help them feel more supported and connected.

Your role is to help people rediscover community, build confidence, and create pathways to a richer and more connected life.

How You'll Make a Difference

Have meaningful conversations with people about what matters most to them

Support individuals to identify goals that improve their wellbeing and quality of life

Connect people with community groups, activities, services and social opportunities

Build relationships with local organisations, community groups and services

Encourage and support people to take ownership of their own wellbeing journey

Reduce social isolation by creating opportunities for connection, participation and belonging

Work collaboratively with families, carers, health professionals and community partners

Help strengthen community networks and improve access to local supports

What We're Looking For

We are looking for someone who genuinely enjoys working alongside people and communities.

You might be someone who:

Is naturally curious and asks thoughtful questions like "What matters most to you?"

Believes in a strengths-based approach, focusing on people's abilities and interests

Is creative and enjoys finding new ways to connect people with opportunities

Feels comfortable building relationships with community groups and local services

Believes that community connection is essential for health and wellbeing

Values compassion, dignity and respect in every interaction

Enjoys working collaboratively while empowering people to make their own choices

Degree in Social Work or another Allied Health discipline

Experience working in community services, mental health, or social support environments

Strong communication and relationship-building skills

Current Driver's Licence

NDIS Worker Screening Check and Working With Children Check
