Job Description
To provide long-term, personalized support to individuals experiencing chronic homelessness through the Journey to Social Inclusion (J2SI) program.
* Deliver case management services to a small caseload using a trauma-informed and team-based approach.
* Help clients access stable housing, build life skills, and foster independence.

Required Skills and Qualifications
Candidates must have:
* A qualification relevant to community services such as social work, psychology, or occupational therapy.
* Demonstrated experience in case management.
* A valid driver's license to drive in Australia.
* Ability to restore relationships with people experiencing vulnerabilities.
