The Sydney Children's Hospital (SCH) – Lead Pharmacist Transplant & Cellular Therapies provides high-level, evidence-based, tertiary clinical pharmacy services to paediatric patients undergoing cellular therapies, including autologous and allogeneic haematopoietic stem cell transplantation (HSCT) and CAR T-cell therapy. This position involves comprehensive medication management, clinical pharmacy services, and care co-ordination, tailored to the complex needs of children receiving cellular therapies in an inpatient and ambulatory setting. What you'll be doing
The Sydney Children's Hospital (SCH) – Lead Pharmacist Transplant & Cellular Therapies provides high-level, evidence-based, tertiary clinical pharmacy services to paediatric patients undergoing cellular therapies, including autologous and allogeneic haematopoietic stem cell transplantation (HSCT) and CAR T-cell therapy.
This position involves comprehensive medication management, clinical pharmacy services, and care co-ordination, tailored to the complex needs of children receiving cellular therapies in an inpatient and ambulatory setting. The Lead Pharmacist will work closely with the transplant, haematology, and immunology multidisciplinary teams to optimise patient outcomes in alignment with hospital protocols, FACT-JACIE standards, and the NSW BMT & Cellular Therapies Network. This position also provides services to non-malignant haematology.
The Lead Pharmacist will also supervise and lead research, clinical trials, and quality improvement initiatives in transplant and cellular therapies. Including responsibilities for medication safety, governance of cytotoxic and immunomodulatory therapies, and education and training to support the development of paediatric pharmacy capability in cellular therapies at SCH.

Describe a situation where you identified and managed a significant medication‐related risk. In your response, outline how you communicated with patients and families to support medication safety.
