Key Responsibilities:
As a Disability Service Coordinator, you will:
Build meaningful relationships with SIL/in-home participants and their families.
Assess participant needs and create personalised support plans.
Coordinate services and resources to ensure participants' well-being and independence.
Advocate for participants' rights and access to community resources.
Monitor and evaluate progress, adjusting support plans as needed.
Collaborate with a diverse team of professionals to provide holistic care.
Maintain accurate records and documentation.
Effectively manage participant schedules and rosters.
Play a pivotal role in assembling and enhancing participant support teams.
Foster strong relationships with allied health professionals, therapists, and specialists.
Act as a liaison between our organisation, participants, their families, and external stakeholders.
Work closely with participants and their families to develop personalised support plans.
Proactively identify opportunities to enhance the quality of services provided.
Roster management of the participants under your workload.
Incident management on a weekly or fortnightly basis.
Qualifications:
Passion for supporting individuals with disabilities.
Strong communication and interpersonal skills.
Empathy, patience, and a positive attitude.
Experience in a similar role within the disability sector is highly desirable.
A relevant qualification in disability services or a related field is a plus.
Strong organisational and time management skills to handle multiple cases simultaneously.
Familiarity with disability-related services, resources, and government assistance programs is an advantage.
A degree in social work, psychology, special education, or a related field is preferred but not mandatory.
