Clinical Care Coordinator – Southern Cross Care (NSW & ACT)
We are a not‐for‐profit aged care provider offering exceptional care for older people. The Clinical Care Coordinator supports Registered Nurses and Care staff by managing clinical delivery, ensuring quality care, and maintaining standards while fostering a positive work environment. This is a full‐time position.
Responsibilities
* Monitor the care plans of residents
* Advise on complex nursing and care needs of residents
* Implement continuous improvement activities to maintain high standards of client care
* Review and update care plans regularly
* Manage residents' condition and progress, ensuring documentation meets SCC guidelines and accurately reflects ongoing care
* Liaise with doctors, paramedics, hospitals, health care professionals, and other specialist service providers
Requirements
* 5 years' experience caring for the aged or in a health‐related area
* Registered Nurse experience
* Strong leadership skills
* Good interpersonal and written English communication skills
* Ability to work positively as part of a Care Team, providing quality care and support to frail aged people
* Numeracy skills – reconciliations and statistics
* Sensitivity to and understanding of the needs of the elderly in a residential care setting
