Our organization is seeking a highly skilled and experienced Care Team Leader to lead our team in Kalgoorlie on a full-time basis.
Key Responsibilities:
* Lead employees to ensure best practice and compliance with policies and procedures.
* Maintain inclusive communication with the team to keep employees informed of key information.
* Allocate suitable resources and implement support plans to deliver quality care.
Care Management Activities:
* Coordinate all care management activities for clients within the Support at Home Program.
* Conduct initial and ongoing assessments to review client care requirements.
Requirements:
The ideal candidate will have a strong background in the Aged Care Industry and be able to demonstrate the following experience, skills and attributes:
* Understanding of healthcare within a community environment.
* Demonstrated knowledge and experience undertaking care management activities within Support at Home programs.
* Experience leading a team to achieve outcomes.
