About This Role
Your new role:
As the Midwife Navigator – Gestational Diabetes, you will provide advanced, autonomous clinical practice to coordinate safe, seamless and person-centred care across the maternity continuum. You will work collaboratively with multidisciplinary partners to improve outcomes, reduce fragmentation and support women to actively manage their health.
Key Responsibilities Include
* Coordinating and navigating care for women with gestational diabetes across services and settings.
* Providing advanced clinical assessment, education and support to promote self-management and health literacy.
* Leading system improvements, referral pathways and care coordination to enhance efficiency and outcomes.
* Contributing to education, quality initiatives and evidence-based practice within Women's & Children's Services.
About You
You are an advanced practice midwife with specialist knowledge in gestational diabetes and a strong commitment to person-centred, culturally safe care. You are confident working autonomously while influencing outcomes through collaboration, leadership and evidence-based practice.
You Will Bring
* Demonstrated experience delivering and coordinating complex gestational diabetes care across antenatal, community and acute settings.
* Specialist expertise in gestational diabetes care and complex maternity care coordination.
* Strong clinical judgement, communication skills and ability to engage women and families.
* Experience contributing to quality improvement, education or service redesign initiatives.

We offer our staff and students extensive support and training in most medical specialties. As a teaching hospital, we help train future health care professionals and participate in research through strong partnerships with Queensland's leading medical schools, universities, and TAFE Queensland.
Our services will continue to grow as we build bigger and better facilities to support the community. By 2027, we'll have more beds and 5 more birthing suites as part of the Logan Hospital expansion.
