The Aged Care Assessor (Clinical) conducts comprehensive assessments of frail older individuals and younger persons with aged-related disabilities, evaluating their physical, medical, psychological and social needs across various settings. They develop personalised support plans that prioritise client's preferences for living arrangements while ensuring cultural sensitivity, particularly for CALD and Indigenous populations. The role also involves implementing Reablement strategies, providing short-term support, managing referrals and maintaining accurate documentation. Additionally, the Assessor participates in triage, professional development, team collaboration and quality improvement initiatives while mentoring new staff. By fostering a client-centred approach, they promote independence and enhance care quality.

To be successful, you will require;

- Bachelor Degree in your related discipline; Social Work, Physiotherapy, Speech Pathology, Occupational Therapy, or Nursing.
- Discipline Specific registration
- A current Victorian Drivers Licence

- A current Working with Children Check (Employee)
- Previous experience in aged care assessments would be highly regarded
