On 1 January 2026, Sunbury and Cobaw Community Health and Nexus Primary Health merged to form a new, unified not‑for‑profit community health organisation, Omnia Community Health. This decision followed an extensive exploration process that confirmed the merger will improve access to safe, high‑quality and sustainable health and community services across the Hume, Macedon Ranges, Mitchell, Murrindindi and Strathbogie local government areas.
Our organisation brings together the deep community connections, shared values and specialist expertise of both entities. Services span general practice, allied health, disability support, mental health, early childhood, family services, aged care and community wellbeing. These services are delivered from sites in Broadford, Kinglake, Kyneton, Romsey, Seymour, Sunbury and Wallan, with additional outreach into surrounding communities.
About the Role
We are seeking a welcoming and organised Intake Navigator to join our Client Access team. This role is a key entry point for people seeking services at Omnia Community Health and plays an important part in ensuring clients receive timely, inclusive and coordinated access to the care they need.
As an Intake Navigator, you will support client registration, appointment booking and referral processing across a range of community health services. You will work closely with multidisciplinary teams to ensure accurate client information, effective triage of enquiries and a positive first experience for every client. Guided by the social model of health, you will help connect clients with the right service, at the right time, in the right way.
As part of this role, you may be required to work across different sites and undertake travel to support service delivery.
The position supports a flexible working arrangement, including a hybrid model with the option to work from home up to two days per week, while maintaining effective communication, availability, and on-site presence as required.

What you will do
* Respond to client and referral enquiries in a professional, courteous and timely manner
* Register new clients and maintain accurate, up-to-date client records using TrakCare and related systems
* Support the intake and appointment scheduling process across multiple services
* Provide clear and accurate information to clients about available services and pathways
* Work collaboratively with intake, reception and service delivery teams to support smooth client access
* Contribute to efficient and effective intake processes through administrative support and continuous improvement
* Participate in team meetings, training and professional development activities
