Job Summary:
To provide professional service of a Disability Support Worker and/or Nursing care to clients based on their individual needs/desires via an established documented care plan. Work directly with the client, their families and friends, reporting to the Nurse Next Door Care Designer ensuring continuity of care. Supporting every client in promoting happier ageing, and providing safe client care in accordance with the Australian Aged Care and NDIS standards and Nurse Next Door policies.
Responsibilities:
* Creates Happier Ageing experiences and client focused care in home and community settings
* Delivers on our Brand Promise by owning the client's experience, ensuring that you do whatever it takes to bring peace of mind to our clients and their families
* Assist clients with activities of daily living: healthy meal preparation, personal care, light housekeeping, errands, companionship, transportation, as required
* Meets safety standards, using equipment safely and accurately
* Provides premium care approach to clients and their family members
* Responds to Care Services Centre (CSC) Team regarding scheduling inquiries and updates within the agreed timelines
* Reviews the care plan prior to all visits, completing records accurately and completing all tasks as requested
* Engages in open communication with your Care Designer when opportunities to elevate your client's experience and care are identified
* Updates availability and leave requests (as per stated expectations) with HR to ensure continuity of care for your client
* Knowledge and understanding of standard infection control principles
* Clinical assessment and documentation skills
* Prepares, administers, and documents all actions taken specific to prescribed oral, topical, subcutaneous and intramuscular medications.
* Observes, reports and records adverse reactions to medications and/or treatment to the Care Designer.
* Provides direct client care as defined under the scope of nursing practice and using sound judgment and critical thinking.
* Recognises the need for and implements emergency actions in appropriate manner, when indicated.

Availability:
* Flexibility: some weekend and public holiday flexibility is required
* Casual role with a range of shifts available
Business Relationships:
* Reports to the Care Designer

Requirements:
* AHPRA Registration or completed Cert lll in Aged Care / Disability or Individual Support
* Community experience
* Two positive professional references
* Police Check (Unsupervised Access)
* NDIS WC and current Blue Card
* Ability to travel independently to client's visits
* Excellent communication skills, both written and verbal
* Ability to complete e-Learning
