The RoleWe have an exciting and fast-paced opportunity to join our Hahndorf (SA) Infinite Clinical Care team as a Clinical Care Coordinator & Nurse Mentor. This full-time role reports to the Facility Manager, with support from the Clinical Care Manager, and involves supervising and supporting our passionate care team.The primary purpose of this role is to ensure our residents in Hahndorf receive the highest quality clinical care. The Clinical Care Coordinator & Nurse Mentor manages care needs, ensures documentation and assessments are up-to-date, and aligns practices with best-in-class clinical standards.Key ResponsibilitiesProvide clinical expertise and lead the clinical and care service teams across the facility.Assist the Clinical Care Manager in leading care teams to meet all clinical needs of residents, including monitoring clinical indicators to achieve optimal outcomes.Manage assessment, documentation, and submission of AN-ACC requirements within the facility.Support documentation, policy implementation, and IT system usage by the clinical team.Coordinate assessment, care planning, and review processes.Support the admission process for new residents, including assessments and consultations with residents and families.Ensure accurate recording of information in compliance with organisational and legislative standards (AN-ACC, resident records, staff time).Contribute to staff rosters, ensuring the right skill mix for effective care delivery.Manage clinical risks and promote continuous improvement within the facility.You will offerInnate empathy for residents and a commitment to improving clinical care quality.Minimum 1 year experience as a Care Coordinator or similar leadership role.Availability to work day shifts Monday to Friday.Current unrestricted Div 1 Registered Nurse registration with AHPRA.Postgraduate qualifications in aged care are advantageous but not essential.Extensive knowledge of aged care assessments, care plans, AN-ACC, accreditation, and documentation.Engaging leadership style capable of influencing and developing clinical teams.High computing skills to manage documentation and reporting requirements.Strong communication and interpersonal skills, with a passion for clinical leadership.Ability to manage multiple stakeholders and handle stressful situations calmly.Valid NDIS Workers Screening Clearance or ability to obtain it.If you'd like to learn more, please contact our recruitment specialist Victoria at
