Murray House Wentworth Aged Care is a community-owned, not-for-profit residential facility (up to 70 residents) delivering high-quality, person-centred care in a supportive, home-like environment.
The Admission, Assessment and Care Planning Coordinator (Registered Nurse Level 5) plays a critical role in ensuring the smooth and timely admission of new residents, comprehensive assessments, and the development of individualised care plans that align with resident needs, preferences, and clinical requirements. Reporting to the Director Care Services for assessment and care planning aspects of the role and CEO for waitlist management, this role ensures that all care planning activities meet regulatory and accreditation standards while contributing to continuous improvement in care quality and resident outcomes.
The Role
* Lead and coordinate the admission and initial assessment process for new residents.
* Develop, review, and maintain comprehensive and individualised care plans.
* Ensure clinical documentation is accurate, timely, and reflective of resident needs.
* Support the clinical team in delivering coordinated, high-quality care aligned with care plans.
About You
* AHPRA registration as a Registered Nurse
* Minimum 3 years of post-registration nursing experience, preferably in aged care
* Demonstrated knowledge of admission processes, clinical assessment, and care planning
* Strong documentation and communication skills
* Current First Aid/CPR certificate
* Current NDIS Worker Screening Check
