FocusOne Health is seeking an Intake and Access Coordinator to play a pivotal role in supporting clients to access and transition smoothly to our general and mental health services. This role works collaboratively across multidisciplinary teams to ensure clients receive timely, coordinated, and person‑centred care.
The Intake and Access Coordinator brings expertise in assessment and service navigation, working closely with designated access teams to facilitate appropriate service allocation and follow‑up across all FocusOne Health programs. By reducing service fragmentation and enhancing engagement, the role contributes to improved client outcomes and a more integrated care experience.
Candidates with relevant qualifications in Social Work, Occupational Therapy, Mental Health Nursing, Registered Nursing and Registered Psychologists are encouraged to apply.
