The primary goals of the service are to: Provide hospital-level care within the community setting. Provide specialist clinical assessment and management of patients in the community setting, allowing for earlier transition home from the hospital environment and/or complete hospital substitution for suitable patients. Provide community assessment and management for geriatric patients under the supervision of a Geriatrician, with support from a comprehensive allied health team. Provide rapid response interventions for acute health issues and chronic condition exacerbations, preventing hospital presentations and admissions. Provide a single point of referral and contact for patients, carers, families and health care providers.
